Insurance

Does insurance cover the surgery if your bmi falls below 40 if you're already in the program but were required to lose weight while in the program? My bmi is 41.2 and they want me to lose 5% of my weight which will put my bmi at 39.1.. Does insurance look at your starting bmi or the bmi at the time of surgery..?
