Meritain Health Insurance

Hello, I just found out that my husband's employer is switching to Meritain health insurance. Does anyone else have this insurance? What are their requirements for gastric sleeve? I'm very nervous because I'm already in this process of going to support groups, specialists, etc.

I will appreciate any info anyone can give me .Thanks!
