Insurance requirements to convert a sleeve to bypass?

One of my concerns with a sleeve is having severe GERD. If I get a sleeve and the GERD becomes too much, I know the best option is to have it converted to a bypass. What are the insurance requirements to do this ? Is it the same as the requirements to have a bypass , or would it be less since I just need to convert a previous bariatriac surgery to another, to alleviate negative symptoms.
