Waiting to see if insurance approves

I was just wondering how the process is from the first visit until the surgery. I have United Healthcare and Blue Cross Blue Shield so I'm hoping one approves. I'm right at 40 bmi.

My BCBS FEP covered the surgery. I think I paid a total of $100 out of pocket.
