I have UHC and I am getting sleeved in Dec. With my policy, a BMI of 40 or higher qualifies you for surgery without the need of a co morbidity. They also required a 5 year weight history, a 6 month medically supervised diet and a psych eval. I had to go through all of the doctor's pre-testing as well.
