My Gastric Sleeve Pre-OP Requirements

So I had my first appointment with my doctor yesterday. I absolutely love him! He's very blunt and to the point, which I found hilarious. I brought my mom with me because she was not supportive of this surgery at all. She thinks I can just diet and lose it on my own, but I have tried dieting for 3 years now, and I always fall back into gaining weight. I am 24 (25 in a few weeks) and I'm sick of wasting my young life being overweight.

My surgeon got straight to the point and said, "This surgery has a 90% success rate of keeping the weight off for the next 10 years. Without this surgery, you will most likely gain the weight back." My mom asked him questions about my Pseudotumor cerebri and he said, "This surgery will absolutely help." He also said it will probably cure my PCOS. That conversation convinced my mom that this surgery is probably my last hope. I am so pleased because now she is no longer rolling her eyes at me every time I bring it up. She's actually saying that my sister probably needs it, too.

Now about the requirements that BCBS has given me... They are pretty stupid. Even my surgeon agrees. He said that BCBS gives us these requirements because they think we will give up trying to get everything together, because they don't want to pay. That is believable.

1. I have to get records together proving that I have had a BMI over 40 for the last 3 years. (This one isn't so bad. Since I have been overweight for the last 7-8 years, this should be fairly easy to obtain.)

2. I have to be put on a diet by a DOCTOR for 4 months. (That means, the 4 months that I have been put on a diet by a Nutritionist are worthless in this situation.)

3. I have to have a Cardiac Evaluation. (This is understandable. They have to make sure my heart is strong enough to handle the operation.)

4. I have to have a sleep Disorder Test. (Also understandable. I'm having one July 29th. They said there's a possibility that I may have Narcolepsy. Let's hope it's just sleep apnea.)

5. I have to have a clearance letter written by a doctor. (This one is kinda stupid. I'm pretty sure my surgeon is more than able to know I need the surgery. I'm worried my doctor may charge me to write this letter.)

6. Psychological Evaluation (This is bogus, too. But I guess it's also understandable. They must want to make sure I'm not an emotional eater. I can already tell that's not the case. I'm just hungry, dang it! Lol. The oral steroids they put me on years ago started this.)

7. I have to see a Nutritionist. (I hope the one I'm seeing is good enough. They are so against me getting this surgery, though. I know it's because they're worried about losing me as a customer. Those scumbags...)

8. I have to schedule a 4 hour nutrition class. (I guess this won't be so bad.)


I have 7 months to do all of this. I really wanted to have the surgery before next year, but I guess it's worth the wait.

Welcome! I am so glad to hear your mom is on board. Having her support will be very helpful.

As to point 7 about the nutritionist. You may want to think carefully about whether or not to stay with your current NUT if they are not supportive. You definitely need someone with Bariatric/WLS experience.

It may seem like there are too many post-op requirements, but they really designed to help you succeed and to have the safest experience possible.

The time goes fast and you can make the most of it to help you succeed long-term.

Yeah, I'm thinking about seeing another Nutritionist. They told me they knew a woman who had the sleeve done and it didn't work. Well, she's probably the 10%, if it's true. They barely ever have customers when I come in. I'm pretty sure they aren't doing good business. They gave me natural Vitamins to help with hunger, and they did not work. I was always starving on their diet. I would get horrible Migraines and feel completely drained.

I really do hope the time goes by fast.

@@Hiraeth I'm happy that your Mom is supportive of WLS. Does your Bariatric Program have a nutritionist? If so - I'd go through them. I had my required supervised weight loss with them and also my initial Eval. I also had my Psych Eval thru my BP. I asked for additional sessions to help me prepare for emotional eating challenges and found that to help the most. I recommend that to anyone going thru WLS. Use this time to learn living post op. I wish I started this at your age!

Hey there - I had BCBS too. They require the 6 month classes which are really good (1 a month and 3 life skills). You learn a lot about living healthy and how to take care of yourself. Each class is about an hour. You get a referral from your primary care or what ever doctor suggested this to you. They send that letter to your surgeon. You don't need written proof of your diets and the surgeon discusses this with you and decides if the surgery is right for you. Then you start your classes, see a psychologist who makes sure you are doing this for the right reasons and mentally healthy, you get a stomach scope to make sure you don't have any other issues, you visit the sleep/lung doc, the heart doc for a check up of your heart to make sure your body will be able to withstand the surgery and the weight loss, then to the nurse practitioner for blood work, then back to the surgeon for your final appt and more blood work and pick up your preop diet materials. After that the insurance company approves your surgery after a letter from the surgeon stating you have completed your requirements.

It sounds like a lot of work but its not and its worth it. It goes super fast.

have BCBS also. From what I see here, most of the requirements are the same regardless of heathcare provider. You should have no issues getting all that done in 7 months. I was trying to get most of my referrals done within a month in Dec//jan, but saw the cardiologist who approved, but he wanted me to see another specialist who cleared, who wanted me to see another. What a money making scheme I thought. I had been on so many diets with my primary that I was able to just provide all those monthly notes from him for the diet portion. I did have a sleep apnea test. You see they more you can prove to them, the better. If you are very overweight and snore, you likely have it. That will be supportive in order to get approved. In the end, I had to postpone the surgery from Jan to August. I did have to get BCBS to reapprove, which thankfully they did. It was bad enough going thru all these hoops the first time around. I do have to get my primary to give me another cleared for surgery letter. ugh

yeah, the psych eval was kind of stupid, waste of money for my part; but from what I read here there are so many who have regrets or bad marriages going into this and it will affect the outcome. My psych eval lasted 15 minutes and we mostly talked about the psychiatrists upcoming knee surgery. $175 later, I had a letter of clearance.

My doctor said these things are not required, except the sleep apnea test and the cardiac eval. He actually called me an hour after typing this and told me he was thinking about me since I left his office. He told me he is going to set up a direct conference with BCBS to see if they will cover my surgery without these requirements, since my vision is getting worse from the Pseudotumor cerebri. I really hope they approve it.

It's so neat to run across someone using the same surgeon and starting their journey around the same time as I am. That is too cool! Did you have your appointment sometime in the afternoon? I walked into the office on the 28th to get a work excuse during the meeting with other patients. I was the short brunette in the red Alabama shirt, lol. I'm so excited to see that someone is starting their process at the same time as me! What do you think of Dr. Miles? I think he's so funny!

Yes! I was there from 3pm until around 4:30ish. I don't remember seeing you though. I was so preoccupied going over the long list of things that I need to do before surgery.. I do like Dr. Miles. I really liked how blunt and honest he was.

Haha! Me too! He's so sweet. He's setting up a conference with BCBS to see if they'll go ahead and approve my surgery since I have pseudotumor cerebri. He seems very worried. I hope we get through this! I'm so anxious to get this done!

That's awesome! I really hope you can have it asap! I have heard that it will be harder to lose weight because of our PCOS. Have you heard that? Kinda makes me nervous..

I have a few appointments set up and I'm excited about going ahead and getting all of my requirements done so that I can move ahead with my surgery as well.

Do you mind telling me what city you're in? I live in Millbrook, and it would be nice to see if you're close by so I can get some recommendations for the required appointments.

I didn't hear that about the PCOS. Wow, that makes me nervous, too. Though, I'm sure we will be fine. I know of someone who had it done, who also had PCOS, and she lost 80 pounds in 4 months. Hopefully, we have her luck, lol!

I live in Helena, but I frequent Birmingham. They're both over an hour away from you. :( It'd be nice if we could be each others support system. I'm still ecstatic that we both are starting at the same time, while using the same doctor, lol!

I'm having my surgery in Birmingham too! Dr Pennington at Grandview Medical Center. Sorry, I haven't seen anyone else on here near me.....I got excited !

      I've hit a stall 9 months out. I'm not worried, though. My fitness levels continue to improve and I have nearly accomplished my pre-surgery goal of learning to scuba dive! One dive left to complete to get my PADI card 🐠
      I was able to go for a 10K/6mile hike in the mountains two days ago just for the fun of it. In the before days, I might have attempted this, but it would have taken me 7 or 8 hours to complete and I would have been exhausted and in pain for the next two days. Taking my time with breaks for snacks and water, I was finished with my wee jaunt in only 4 hours 😎 and really got to enjoy photographing some insects, fungi, and turtles.
      Just for fun last week, I ran two 5Ks in two days, something I would have never done in the past! Next goal is a 10K before the end of this month.

      Hi everyone, I wrote back in May about having no strength. I still get totally exhausted just walking from room to room, it’s so bad I’m using a walker with wheels of all things. I had the gastric sleeve Jan. 24th. I’m doing exactly what the programs says, except protein shakes. I have different meats and protein bars daily, including vitamins daily. I do drink my fluids as well.  I go in for IV hydration 4 days a week and feel ok just til evening.  So far as of Jan 1st I’ve dropped 76 lbs. I just want to enjoy the weight lose. Any suggestions or has anyone else gone thru this??  Doctor says just increase calorie intake, still the same.

      Losing my hair in clumps and still dealing with "stomach" issues from gallbladder removal surgery. On the positive side I'm doing better about meeting protein and water goals and taking my vitamins, so yay? 🤷‍♀️
