What happens after 6 months of pre-op

What if I do everything right in preparing for surgery and get denied by insurance? Had that happened before?

I think you should have confidence in the fact the surgeon would not take you on as a patient if she/he thinks you will be denied, lots of time and resources would be wasted on his end... Stay confident, life has a funny was of making things work out.
