Neighborhood Health Plan of Mass...

Does anyone know if NHP is difficult in getting approvals? I have a BMI of 39.7 I'm having an at home sleep study done but I'm worried if it comes back negative for sleep apnea. Thoughts?

I've been trying to gain a few more pounds. Which weight do they submit to the insurance company? I'm assuming it would be the highest one.
