I got approved with Medicaid. They required 6 months working with my doctor on weight loss but my doctor wrote a letter to the insurance company explaining why he recommended it for me and I was approved right away. It did take several weeks to do my appointments with the exercise therapist and nutritionist and I had to have a psych eval. I think it took 2 months from my first appointment with my surgeon to get the approval and then my surgery was scheduled about a month after that.
