Hey guys!!!
I am so new to this.
So someone called me from the bariatric office to go over all the steps that I need to get insurance approval. For oxford united health I think that you need;
-bmi of 40 and up ( My BMI is 42)
-6 month of documented weight loss ( I am on month three now )
-a psychological evaluation. ( This appt is on the 5/16/2016)
Now she tells me that I need the last 2yrs prior to surgery of doctor records of attempts to lose weight....
I went on oxford united health to see if this a requirement to get approval and I don't see it anywhere. Please see link below.
