I thought I could find a form from my insurance company that would allow me to setup whatever it is I might need for my surgery, but I can't find a thing on their website under their forms.

I was wondering if anyone had any advice or information on what I might be able to do to set this up? I totally forgot I needed this and imagine I am making more of it than it is, but any help would be greatly appreciated! Thanks!

Check with your surgeon's office or the hospital. I was able to get a living will form from the hospital where I had my surgery.

Your attorney general website has information. I attached the link below. Hospitals and doctors offices should also have the forms that you need. Make sure you keep the original and give copies to your proxies, the hospital and the doctors office.
