Don't have that insurance. I would suggest you call them and ask what is required for weight loss surgery.

They will be you're best bet in determining the requirements. Prepare yourself because most insurance companies require a lengthy supervised diet before approving.

I will say that all the hoops you may have to jump through will be well worth the effort. I had medica insurance. It took 9 months to get through the whole process. My insurance required a 6 month supervised diet but it took longer to get through everything.
