Blue Shield CA not paying for pre-surgery requirements

My insurance has several requirements including a psych eval,6 months supervised diet/exercise. My surgeon also has ordered tests a test that has not been covered - the one where you swallow barium and the pop rocks to make your stomach poof up. BCBS has denied the charges for these services. Any advice?

that just doesn't sound right to me. I have BCBS, but MA. They have paid for all of the appointments and test so far (we co-pays).
