Supervised weigh-loss requirements for BCBS of Iowa

I have BCBS of Iowa and have a BMI of 38.4 with two(2) cormobid conditions. I am going through True Results of Houston, and was told I have to meet insurance requirements of several tests and procedures to get the surgery approved. I was told I have to do 6 months of supervised weightloss visits.

When I call BCBS, they are saying "No more than 6 months of Supervised " and it is based on the doctors choice.

I had another doctor tell me 1-3 of supervised weight-loss visits.

Any one experiencing the same thing?
