United Healthcare Choice Plus Point Of Service

Does anyone have any experience with this plan? I'm switching to it effective January 1st. I know it covers bariatric surgery but I don't know what guidelines need to be met yet.

    Each policy is different and it can also vary from state to state. In general UHCs policy is pretty open. You must have a BMI of over 40 for a period of at least 5 years and demonstrate that you have tried other options and these have failed. If your BMI is under 40 then you need to have specific co-morbidities. If you search United Health Care Bariatric Surgery Policy you can read it and see if there are additional qualifications for your state.
