Contacting Dr Ramos Kelly, any info?

I was sleeved on May 10th 2012 by Dr Ramos Kelly. At the time the coordinator was Omar, who was fantastic. I wanted to get back in touch with Dr Kelly or his coordinator if anyone has an email address to contact him please!
