Already had surg, just got confusing letter from UH

I had surgery in Dec. & was approved by UH. Yesterday, I got a letter from UH saying they had received my appeal to their denial of procedure on the date of my surgery.

Anyone have this happen to them before? I will call tomorrow but it's very confusing.
