BC/BS Florida

Has anyone had Bc/BS of FL pay for their lap band surgery? I am currently on a six month dr supervised diet which is part of their requirement for consideration, but I have yet to hear of anyone who has ever been approved BC/BS FL for Lap Band surgery. If any has gotten approval, please let me know. Thank you.
