What reasons were you denied whether it was the first time second time etc? What did you have to do to get the insurance to reverse their decision?

I guess no one has ever been denied for insurance? This topic does not seem to be very popular. I see in other topics where people talk about being denied, but they never really say why they were denied or what they did to change the insurance companies decision.
