Help understanding the process of getting a gastric bypass through Molina.

Can anyone help me get through/ understand the process with Molina Healthcare? I am totally confused & frustrated bc I can not find anyone with the answers I need. I've had the initial form completed and faxed ( back in June,), & have not heard anything yet. I just want to know what I need to do next.
