Self pay time frame and preop requirements

Looking to have band to sleeve revision, will be self pay. Would love to find a surgeon in the Huntsville, AL area. Any suggestions would be helpful! How long will it take from first appointment to surgery date, what is required for self pay as far as preop tests, psych evaluation, preop supervised diet etc
