Can't provide 5 yrs of weight history

So, I have a question for those of you that have already been approved. My surgeon's ask my PCP for 5 years of weight records. Well, I can only come up with 4. I have reached out to the surgeon's office to see if this was a problem. She told me that most companies only actually look at the last 3 years but she would look in to the specifics of my insurance and get back to me. I am a nervous wreck and would hate to have come this far for me not to get approved over 1 year.....I have 2017, 2016, 2015 & 2014....does anybody have any experience with this situation? I have Tricare Reserve (standard).

I am suppose to get sleeved in early July....I don't want any set backs now!!

Getting and waiting for approval is so nerve wracking and I feel for you. Each insurance co is different. Mine never had me document that many yrs. My PCP had me try pills so that was on record but then just listed I had tried WW, TOPS, etc. I sincerely wish you the best and hope there's no delay for your July date. Hopefully they will let one year slide. Good luck.
