Brachioplasty...Anyone have this covered by insurance?

I am trying to get my insurance company (whom I work for) to cover my brachioplasty. It has been denied and then I appealed and it was denied again. They keep saying that its cosmetic but I've submitted proof of rashes, rubbing, length of hang, etc and still nothing.

I have the option of a 2nd level appeal but I'm wondering if its even worth it. Suggestions?
