fills and band size

I am really confused. I went on Saturday for my fourth fill - was seen by bariatric nurse rather than doctor and she told me that I was at my maximum for my band - apparently I have a 10mil band and I have 4m in it
How is this the maximum?
Also I had great restriction after my third fill for a couple of weeks - not really lost any weight though as the 6lbs I lost I put back half of it. So wanted a fill so that I had restriction again.
She put .02 in - I think just to let me think I had not wasted my journey but haven;t got the restriction I had after my last 'big' fill.
So how the hell do I lose this last stone[14 lbs]??????????
