I can't seem to find any one with this unsurance who's had a sleeve. I'm in NH which is one of the state's where it's required that employers offer the bariatrics in the insurance, so that's a bonus. I checked on the ins Co website and it IS covered, looks like the norm, 35 with comorbids, 40 without. I'm 40, without. But that's it, it doesn't go into any detail about requirements , any one have any idea?

call them. i have them (live in nh but insured in ma). they did not have anything additional to what the hospital running the program had. i am currently awaiting their review and hopefully approval. i had 1 general info session, 1 nurse meeting, 1 social worker meeting, 2 nutritionist meetings, 3 group nutrition meetings and had to lose 5% of my weight. good luck!
