Ketosis - How safe is it?

I am a bit confused about ketosis, as well. Being a type 2 diabetic myself, I did a search about "ketosis" vs "ketoacidosis"; they ARE different, but you may need a specialist to explain it to you more thoroughly.

You can also read about it from reputable websites. Basically it happens when you eat less than 40 to 50 grams of Protein a day for several days. Your body burns more fat for energy at that point. I figure more of us are in Ketosis than we probably realize because many of us are limiting our carbs.

You and I sound very much alike, @PixelToph. I lost 99 pounds pre-op following a low carb/high protein/high healthy fat diet. And I am going right back to that once I can bring meat and veggies back into my diet. I have a carb addiction issue, and I just have to stay away from the little bastards. (And all of my labs were in the normal range at the time of surgery just from doing that.)

I feel really sorry for so many people on here who go straight from eating anything they want to a very long and difficult pre-op liquid diet to all the restrictions after the surgery. It is a very tall order. I think it is criminal that surgeons don't require people to do at least a month of a low carb diet before the liquids kick in pre-op. My pre-op diet changed my relationship with food -- and that was a necessary ingredient to my overall success in this journey. It sounds like you were the same way!

Thank you! We do seem to have a similar journey. It's good to see another person who started at a higher weight, since it seems RNY is more common for people with as much to lose as us. My surgeon actually had to verify my starting weight when I met with him right before surgery. He thought it might have been a typo since it's pretty uncommon to lose that much before surgery lol. When I told him it was right, he was delighted and said it really bodes well for my post-surgery success. I also didn't have to do the 2-week liquid pre-op diet, he said with the weight I lost my liver was already smaller, so it wouldn't make much difference. I just had to increase to 2 Protein shakes a day, and only a light meal the day before surgery.

Could you share some of your vegetarian keto/low carb meals?

I don't like cooking much, so I didn't do anything special. I mostly took meals I already ate and made them keto-friendly. I'm hispanic, and tacos/burritos were a common meal, so I make taco salad instead of having tortillas and skipped the rice completely. I also switched to hand seasoning instead of using those seasoning packets. They can be surprisingly carby (cornstarch added, sometimes sugar too). I saute Morningstar crumbles with ground chili powder, cumin, garlic and onion. Serve over some lettuce and top with cheese.

We also enjoy a good Indian curry, so I just made sure to check the sauces for sugar. Quorn makes a great veggie-chicken and you can get it either in pieces or cutlets, just make sure it's not the breaded version. Paneer cheese, diced "chicken" and spinach simmered in curry sauce is really tasty and a one-pan dish.

Now that I'm post surgery I don't have room for sides, but pre-surgery mashed cauliflower makes a decent substitution for mashed potatoes.

I'd like to follow this please - I'm very interested in how it effects people - I'm diabetic but believe I went into this today - well I had low blood sugar so maybe its not the same thing... anyway - I ate carbs as I felt it was what I needed at the time...

I'm diabetic too...I am a bit worried about this because of diabetic ketoacidosis, but I read that ketosis is different than that. This is an article I came across...seems legit:

I would talk to your doctor to make sure it is safe for you. For most it is very safe but there are exceptions to that and only your doctor knows your underlying health conditions. Assuming it is safe it worked wonders for me. I got to goal weight in about 7 months. I am no longer in ketosis but most of my carbs are veggies, fruit and some potatoes. No wheat sugar or rice. I still keep my carb lower than the average person and Protein first. Beating the carb addiction has changed my life!

So good to know that beating the carb/sugar addiction is possible!

I have been doing low carb/high healthy fat diet for 4 weeks now and have lost 22 pounds. I have not had cravings for carbs. Even on those days when my family has higher carb Snacks i can avoid them and still have no problems. My PCP is following my pre and post op care since i am having sleeve in Mexico and this is the diet she requested I be on.

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
