Colorado Medicaid questions

I am starting the gastric bybass journey and wondering if anyone knows the out of pocket expenses in Colorado. I called Medicaid and the most horrific mean woman did not help and and was so mean I'm not calling back. Does anyone know what the requirements are to submit par for surgery? Does Medicaid cover the six month weight loss management prior to authorization

I live in NV so it could be different but all of my Dr visits have been covered and my testing preop (blood, ekgs, stress test, psych...) the only thing Ive paid for is the nutricianist once a month. Medicaid paid for all that then denied my surgery :( working on appeals now
