5 years of medical records?

So, a requirement by my surgeon is to have 5 years of medical records - which I don't have. My coordinator let me know since I didn't have health insurance for a while and have not visited a dr. in a long time that I can write a quick letter letting them know why I don't have any records.

Did anyone else have to do this? I'm not sure what to write or how long it should be. I am in need of some input!
