In 2016 I'm will be on my husbands insurance and it covers up to 80% for Bariatric Surgery. Does BCBS Anthem PPO require you to do the 6 -12 month supervised weight loss? I'm trying to get a primary doctor now to start if that is the case.... Does anyone have any information on this or what steps do I need to take? I'm looking forward to healthier 2016.

Yes - they require a six month medically supervised diet program, nutritional evaluation, psychological evaluation, and PCP clearance letter.
