Anthem BCBS Ohio

I know that reading others posts helped me a lot so I wanted to post some info. I got approved by Anthem BCBS Ohio in 24 hours. My doc submitted 6 monthly diet forms along with my psych evaluation. My BMI is 43.

    I have Anthem BCBS Ohio and thought I had 6 months but docs office shows it as 3 months. So nervous there will be a mistake but they had it printed to show me. This worries me but so happy for you!
