What are the steps for band to sleeve revision to be covered by Insurance

I am miserable with this band....I want a revision to bypass.I have Horizon NJ Health.What is the process for approval..I have gained all the weight back,acid reflux,hiatal hernia,PCOS,post breast cancer,pre diabetes,high cholesterol...lithe list continues....I have Horizon NJ Health mercy.
