Approval based on visits to weight loss clinic?

I have United health choice plus in az per my employer and my requirement is 6 visists in 2 years and they dont have to be consecutive which i already have because I have been going to a weight loss clinic since last may 2014, however i didnt go every month. which they said it does not need to be consecutive. was anyone approved in this circumstances ?
