Insurance and surgeon have conflicting requirments!

I feel like I get good news followed by bad news through out this journey! Called my insurance to check on my.max out of pocket I was expecting it to be several thousand cane out to $404. Great but when going over requirements for surgery I was told I have to have 6 months of nutrition and structured excersise program. My dr. Office says no I don't need any. I'm so frustrated:(
