I'm just starting my journey and was wondering what BCBS and Medicaid's requirements are to be approved? Also what kind of out of pocket costs, if any, might there be. Any info is greatly appreciated

Hi I have BCBS Michigan .... They required a 6 month medically supervised diet, also I had to do a stress test and sleep study ( I can't remember if that's required from insurance or physician)

I called BCBS and my surgery is covered at 50% and 100% after my max out of pocket has been reached

My max out of pocket is $5,000 a year. So I can be responsible for up to $5000.
