and it's still Pending but the rep told me that they have my surgery date as December 2nd. Is that a true date or did the surgery coordinator have to just put any date in?

I have heard that they have to put a date in when they submit to insurance, but that that date is/can be later revised once the approval is in.

My coordinator put in an expected surgery date when she submitted to my insurance. However there was some back and forth questions and my actual date ended up being later, based in when the actual approval came in.

My surgeon put in a date of Dec. 17th, 2013, but then my insurance denied it. Let the appeals begin...
