Anyone With Anthem Bc Ppo Have Ps Approval??

I just started the pre-auth process for a panniculectomy w/breast reduction and lift. Anyone out there have this approved through Anthem BC PPO and if so, how long did it take to get approval? Did you have to appeal to get approval?
