Please recommend a Psychiatrist for eval in RI/MA for second surgery

I had the lap-band done in in 2005 and have had nothin but problems with it:(. It has gotten so bad that IF I have even 1 cc of fill, I choke at night since food is getting stuck in my throat. The Dr has decide to remove it and go with the Bypass BUT I need another Psych Eval (Again) but the 2 places I have call either want 4 hours of my time (RI Hospital) and another DR in Norwood, MA wants me to take a 500 question test (Insurance doesn't cover the test) and the 1hr Eval (insurance covered).

Back in 2005 I went to Boston University for the exam, it was easy, in & out very easy. 1 hr exam and that was it.

FYI, I don't remember the person I went to and I have Google and still nothing and the DR doesn't have the record for the Psychiatrist DR either.

I need someone to give me a basic exam, I have been through it so I know what it takes, blah blah blah. I know they are DR out there that can do it 1hr.
