Psychologist or psychiatrist in the Chicago area

Hello,

I am looking for a psychologist/psychiatrist in the Chicago area who could provide a psychological evaluation for LAP-BAND®® surgery. I will be paying out of pocket for this service, so if anyone could recommend someone not too expensive that provides this type of service that would be great.
