Anyone with Western Health Advantage?

Howdy y'all.
I'm trying to find out the qualifications for getting a lap-band with Western Health Advantage.
I know that with a BMI of 35-39 you must have comorbidities... but how many and what is/is not considered a comorbidity?
When I've called and spoken with a WHA rep, they gave me the run around. I asked for and was send a Evidence of Coverage booklet, but again, it really didn't go into any detail. I spoke with my PCP and he was having trouble finding out all the qualifications.
I then emailed a WHA rep and told them I was getting the run around. They told me "Contact your PCP and if your PCP doesn't know, he should contact his Medical Group."
Anyone out there have any ideas?
