I have regained 50 lbs over four years. since Jul 28, 2010, have been on 1100 calories and exercise. Never added starches, carby foods.... My metabolism is shot, and cannot lose weight. I am seeing surgeon about Sleeve Revision to RNY or DS.

its one thing not to lose weight by being in such a strict regiment

its another to gain weight!!! :angry:

sorry you have these severe weight issues :angry:

hope the doc and you have a good discussion

if doc agrees that more surgery would be a good idea, go for it

Can't one see a doctor for the metabolism? Otherwise this sounds like a future scary.

my doctor recently told me to eat small portions always....otherwise slowly the sleeve can stretch..

now that is a scary thought....so better to follow doctor's orders,, she said you can eat every two hours...but eat small portions.

Well the truth is that every sleeve stretches. It would not be possible to eat enough to get nutrition after the DS, the second part of the operation, for which VSG was originally assigned. For some not a problem, for others re-sleeve or revision is an increasing reality.

I would be inclined to go with the DS, as it's the more straightforward revision (at least to a DS surgeon, where it is basically completing the DS) and it offers the best shot at countering the metabolic damage that you have suffered. With the RNY, the caloric malabsorption dissipates after a couple of years, so it may help you get the weight back off in the short run, but in the longer run you will be back where you are now, trying to get by on 1100 calories.

Another thing to consider is that with the VSG, the revision to the DS is straightforward, and something that most any qualified DS surgeon can handle. However, if the RNY doesn't work out and you ultimately need to revise that to the DS, that's something for which only a handful of surgeons in the US are really qualified (maybe a half dozen.) That assumes that the RNY doesn't permanently screw up the sleeved stomach in making the pouch - I know quite a few who have had the RNY/DS revision (and even a couple who started with a band, revised to RNY, revised to DS) but don't know any who have gone the VSG/RNY/DS route yet.

Trying to wrap my mind around this. You eat 1100 calories of good food (not carby or starchy), exercise and still regain 50#? What kind of exercise plan do you have going? Is your sleep good? Do you drink lots of Water and keep alcohol to a min? Is it true you are eating smaller meals over that 1100 calories or not? I did not get that out of the thread unless I overlooked it. Small to me means 200-300 calories at a time which means 4-5 oz Protein and perhaps 1-2 oz of veggie. What are your overall macros? Something like 50/30/20 (protein, carb, fat) or 40/30/30 or something else? If you are doing these things then Yikes. That is a scary scenario. Have you considered increasing calories? 500 below your targeted tdee will mean weight loss, but 1000 (or more) below will mean "starvation mode" and metabolic issues long term. Most of the time metabolic issues can be fixed by making calculated changes to diet, exercise and lifestyle - without something like a revision. Hormones need to be brought back into balance for proper metabolic response. I do hope you can find resolution.

      I've hit a stall 9 months out. I'm not worried, though. My fitness levels continue to improve and I have nearly accomplished my pre-surgery goal of learning to scuba dive! One dive left to complete to get my PADI card 🐠
      I was able to go for a 10K/6mile hike in the mountains two days ago just for the fun of it. In the before days, I might have attempted this, but it would have taken me 7 or 8 hours to complete and I would have been exhausted and in pain for the next two days. Taking my time with breaks for snacks and water, I was finished with my wee jaunt in only 4 hours 😎 and really got to enjoy photographing some insects, fungi, and turtles.

      I have no clue where to upload this, so I'll put it here. This is pre-op vs the morning of my 6 month appointment! In office I weight 232, that's 88 lbs down since my highest weight, 75 lbs since my surgery weight! I can't believe this jacket fit... I am smaller now than the last time I was this size which the surgeon found really amusing. He's happy with where I am in my weight loss and estimates I'll be around 200 lbs by my 1 year anniversary! My lowest weight as an adult is 195, so that's pretty damn exciting to think I'll be near that at a year. Everything from there will be unknown territory!!
