A female member asked me to post this question to Men here on BP as she wanted to respect the guideline of women not posting in this subforum. She also wished to remain anonymous, so here is a question from her:



I'm wondering if any of the men on here had trouble with ejaculation (ie: not being able to ejaculate or having delayed ejaculation) when they were heavier. My husband has been between 300 - 340 lbs (his weight fluctuates) in the two years that I've known him and he has a lot of trouble in this area. Though he gets and maintains an erection the entire time we are intimate, he's never once ejaculated. At first I thought maybe he wasn't attracted to me, but he initiates sex and becomes aroused when we're close.



When he "stimulates" himself, it takes him anywhere between 15 - 25 minutes (and that's AFTER 40 minutes of foreplay and sex with me). My husband and I are pretty young (28) so I don't think it's an age thing. We've been to the urologist who said he'd never heard of this issue and didn't have any advice. My husband has sleep apnea and only wears his mask sometimes. According to him, he has never been abused, nor does he have any feelings of guilt or think sex is bad.



He's been to his primary care physician and aside from needing to lose weight, they found him physically healthy.



I'm at a loss here. Husband's been considering applying for WLS in October if he wasn't lost weight on his own and I'm wondering if this is an area that might improve.

I'm surprised your urologist has never heard of this issue and didn't offer any advice. Did he check your husband's testosterone levels? This is one of those things that tends to snowball. I know it did for me.

I had a very similar story to your husband, though I was a few years older. I was having this issue and to compound things came the mental aspects of performance anxiety, which my urologist believed to be the main, continuing factor. Shortly after my VSG procedure, he did a testosterone test and sure enough, my levels were low. The reference range is somewhere between 350 to 1,200 and mine was 120. I also had what was discovered to be a benign growth on my ejaculatory duct removed. Having this procedure and being on TRT (testosterone replacement therapy) has been the difference maker for me. My levels are monitored and have stayed steady in the 600 range. I was hoping with my having a normal BMI, my levels would return on its own, but it hasn't happened, yet. The TRT may be attributing to not returning on its own, so we will be looking at titrating off.

That is my story. Sexual issues with obesity are common, from my research. It could be physical or it could be psychological and most likely, a combination of both. You are correct in thinking it is not about you, because most likely, it isn't. I would suggest finding another urologist because there are many things that can be done including a cystoscopy. Google it. It isn't as bad as it sounds. But surely your husband knows there will be many benefits to lowering his BMI, including benefits in the bedroom. Have you had him read this sub-forum or others here?

I think this is just something that varies from person to person. Some guys can finish in minutes, some have a hard time finishing at all with a partner. I have had the issue my whole life. Most times it is takes a lot of work to finish when I am with my wife. Before my WLS, it was even more difficult because I would run out of steam alot quicker.

I'm with Pdxman, he needs to get a full blood panel done to check things out. I was low T prior to WLS, around 330 total T at age 26. Some Drs will say that's within range, or "close enough" but if he goes to an endocrinologist, they should aim for you to be at a higher level based on age.

Also, some guys just go...well...longer. My wife and I can't have quickies. If we do, its strictly for her, and afterwards we'll come back later to complete the task.

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
