Medical clearance from PCP

Are most of you required to get medical clearance from your PCP before the band removal? Is this an insurance requirement or a surgeon requirement? My dr office throws this at me today in a letter when she called me last week. It's just another step and another thing they failed to tell me about. So frustrating

    Unsure. I do not recall being asked for PCP approval, but I got it to document weight loss attempts anyway!
