This is thru the obesity coalition.. you have to have your drs office fill out the form for the most part and fax it in.. BUT you get a huge bottle of calcuim citrate chewables (500 i think).. Vitamins.. b12 sublingual.. and Iron. All thru Bariatric Advantage and bariatric approved. Did this today for myself.

I can't find form

Go to obesitycoalition.org.. at the very bottom theres a tab that says recovery form.. click on that itll take u to the page u need to print then fill out

The link is http://www.obesityaction.org/. In the middle of the page, a small way down, you will see an icon (not a tab) for RECOVER PROGRAM. Click on this, and it will take you to the page that has the PDF form.

This is not for samples, as best I can tell. It is for assistance with Vitamins for those that cannot pay, or it would be a financial burden. Its worth a try though! Thanks for sharing!
