Please tell me your BMI and Bougie Size

Hi, I was 40.5 and am now 22.something. My approx bougie size was 40. The bougie is the guide they use to staple round to shape your new stomach. The higher the number the greater the diameter of your tubular stomach in cross section. I was told my stomach should have a capacity between 125 and 150ccs, if that is any help to you all.

Hello Jane, The reason I asked the question is because I read that the larger the bougie the less problems you have with acid upset. Did you have any problems?

I just talked to my surgeon about this today. It sounds like there is alot more to it than just the size. I have a 40, but lately he has been doing "loose 38s" He said it is a percentage of 12 mm difference. so barely any. I have great restriction. I did have acid reflux early out. It seems to be diminishing lately. I no longer take a PPI, but I do get heartburn on occasion (2x/week?)

My surgeon said that he worries about anything smaller than a 38 and making the lesser curvature of the stomach too narrow, potentially leading to a lager pouch at the top and/or near the bottom.

Mostly what I took from his info is that there is alot of technique involved, not just a bougie of xx size.

And to answer the initial question, by starting BMI was 43.

Hi Christina, not now on the whole, but I did take a PPI for about a year. It was pretty uncomfortable for 3 weeks when I stopped, I had already gone from 30mg daily to 15mg daily, but then it all went OK. I took loads of gaviscon in those weeks, avoid tums and Calcium carbonate based remedies because they trigger something called acid-rebound, which ultimately makes the acid worse. before the op I had dreadful indigestion and reflux, but i just lived with it, now I am fine except occassionally I eat something that disagrees and sets off heartburn. I think the issue about acid after the surgery is more to do with the pressure from below in the stomach and the strength of the muscles between the stomach and the oesophagus stopping the acid from rising.

That was a great video. I did lots of research for my appeal and concluded that the largest I would be comfortable with would be a 36f. But I also noticed how much variation there is from surgeon to surgeon depending on how tight they pull the stomach, where they start, and whether they buttress the staple line with oversewing or other methods. In the end, I felt the surgeon I chose was best qualified to decide based on his experience and data. I went with Dr. Aceves and he uses a 36 with an oversew that makes it closer to a 32 or a 34. I have a lot of restriction and think it will work fine for me.

In the end if the differences end up being a tablespoon or two then it really is up to me to use my tool to be successful regardless of the bougie size. I love that people seem to have sleeves from 32 up to 40 and still do just fine.

What I wouldn't have known before surgery is how big the lack of hunger would be for me. While I love the restriction and think it's what will help me re-reach my goal, it's hard to say that a lack of hunger wouldn't have worked for me all by itself. I understood restriction from having had a band but I really underestimated how big a deal the lowered production of ghrelin would be.

I'm too early in the process for my BMI to be helpful when considering bougie size :-).

When I finally had my mind made up I had gotten my BMI down to 32.6 but it had taken me a year to get there and was starting to gain again. I am not sure how big my Bougie is but I know it only hold 3 oz. I know it is scary pre op to think of only eating 3 oz and being happy but honestly it is wonderful! I wouldn't have any other way. I dont have a VSG though I have the TGVP.

Hello Mellifrits, I will have my surgery Aug 18th with Dr. Almanza. I will talk to him about what the best size for me would be. I'm a little worried about losing too much weight and acid reflux.

I depends on what I eat but I haven't had much heart burn at all and I'm sure if I was better about taking my acid reflux meds I wouldn't have any at all. I guess I was scared that since I'm 23 I'm going to have this new stomach the rest of my life and in time it might get a little bigger. So I wanted to start out small in hopes that when I'm in my 40 I will still have my smaller stomach. But I would just talk to your dr about it more. I'm only a month out and he has a lot more insight than I can give you.

I think your doc is the best source of info on what size you should have. I really don't think telling him how to do his job is going to win you any brownie points. He has his reasons for size--he didn't flip a coin.

As a nurse who has been in the medical profession for 15 years, I couldn't disagree more. A well-informed patient who has the intelligence and self-confidence to research and question their treatment, tends to get the best treatment. Those who have well-informed and inquisitive patient advocates also get the best treatment. The idea that people should be more informed consumers when it comes to buying a car than in undergoing a major procedure on their own body is incredible. Why on earth shouldn't a patient question their doctor??

As far as "brownie points" go, one doesn't go to a doctor to seek approval, but to accomplish a healthcare goal. "He" (umm... sometimes "he" is a she, by the way), may have reasons for his choices, but if those reasons don't jib with me, I'm going to trust my instincts and go to another doctor. Doctors (although they sometimes think they are), are not omnipotent. I counsel my patients to ask, ask, ask. This is 2010, not 1952.

:tongue_smilie: I work in health care too and I wholeheartedly agree. Also, people love to tell people to just follow their diet plan, etc. But it frustrates me that there so many different plans out there for the same surgery! Better to ask and fully understand than just blindly follow, IMO.

Thank you Calgarygirl! I love all the great support and advise I get from here. I feel like I have a whole new set of friends. It is very touching how everyone checks on each other.

This week has been dragging..LOL. So today im a little scared. I am having my surgery done by DR Almanza. Im not just scared of the surgery, Im scared of the emotional side too. What about you? How are you feeling?

Mellifrits, I wish I had this done at 23. You look great! Your so young still and now you can enjoy life like a 23 year old should. People keep telling me to work out and diet. I tell them I have been fighting my weight my whole life. Yes, sometimes I lose 15,25 pounds but it just gets harder. Im at 215 now I know for a fact if I didn't do this now I will weigh 230 by 40 and maybe 300 by 50. They don't understand how much harder it will be then. If I would even live to see 50.

Youre funny Christina, you will still be living when youre 50!! lol And I couldnt agree more (jealous of mellifrits) getting this done so young..... I wish I would have instead of doing the yo yo dieting for so long!! ohh wow, so how am I feeling, oh my gosh... every feeling in the world... at first i was worried about the surgery, now Im just worried what I need to wear at the hospital... Im scared, excited, not sure how my family is going to handle this one, and really happy reading everyones stories... Im like thank god I found this lol

But yes youre right - the emotional side!! its a lot of changes, but the vertical sleeve will help make us more aware... I dont know whats its going to be like until we get to that point... do we go running instead of eating?? lol I dont know... Im hoping we have lots more energy and I seriously cant wait for the new me!! Im looking forward to watching your progress, feel free to add me as a friend, and I agree, the support on here is amazing!!! Good luck this week!!!!

Oh yes the working out part...lol...I went to a Water park this week with my two kids, we walked up a million steps and walked and walked. Lets just say I thought I might need major pain killers the next couple of days...How am I ever going to work out..OMG

      2 days until I fly out to San Diego to have my Bypass Surg. in Tiajuana Mexico. Not gonna lie, the nerves are starting to surface. I don't fear the surgery itself, or the fact that I'm traveling alone, but its the aftermath that I'm stressing about the most, after this 8 week wait. I'm excited to finally be here, but I am really dreading the post surgical chapter. I know its going to be tough, real tough and I think I'm just in my head to much now that the day i here. Wish me luck, Hopefully I'm one of the lucky ones, and everything goes smoothly. Cant wait to give an exciting update,. If there is anyone else have a June bypass or even a recent one, Id love to have someone to compare war stories with. Also, anyone near San Antonio Tx? See ya soon with the future me. 💜

      I am new here today... and only two weeks out from my sleeve surgery on the 23rd. I am amazed I have kept my calories down to 467 today so far... that leaves me almost 750 left for dinner and maybe a snack. This is going to be tough for two weeks... but I have to believe I can do it!

      Hey everyone. I'm new here so I thought I should introduce myself. I am 53y/o and am scheduled for Gastric Bypass on June 25th, 2025. I'm located in San Antonio, Texas. I will be having my surgery in Tiajuana Mexico. I've wanted this for years, but I always had insurance where bariatric procedures were excluded. Finally I am able to afford to pay out of pocket.  I can't wait to get started, and I hope I'm prepared for the initial period of "hell". I know what I have signed up for, but I'm sure the good to come will out way the temporary period of discomfort and feelings of regret. I'd love to find people to talk to who have been through the same procedure or experience before. So I look forward to meeting you all. Hope you have a great week!

        I'm so happy for you! You are about to change your life. I was so glad to get the sleeve done in Dec. I didn't have feelings of regret overall. And I'm down almost 60 lbs. I do feel a little sad at restaurants. I can barely eat half a kid's meal. I get adults meals often because kid ones don't have the same offerings at times. Then I feel obligated to eat on that until it's gone and that can be days. So the restaurant thing isn't great for me. All the rest is fine by me! I love feeling full with very little. I do wish I could drink when eating. And will sip at the end. Just a strong habit to stop. But I'm working on it! You will do fine! Just keep focused on your desire to be different. Not better or worse. But different. I am happy both ways but my low back doesn't like me that heavy. So I listened (also my feet!). LOL! Update us on your journey! I'm not far from you. I'm in Houston. Good luck and I hope it all goes smoothly! Would love to see pics of the town you go to for this. I've never been there. Neat you will be traveling for this! Enjoy the journey. Take it one day at a time. Sometimes a few hours at a time. Follow all recommendations as best you can. 💗

        Thank you so much for your well wishes. I am hoping that everything goes easy for me as well. We don't eat out much as it is, so it wont be too bad in that department. Thankfully. Also, I hear you regarding your back and feet!! I'd like to add knees to the list. Killing me as we speak! I'm only 5' so the weight has to go. Too short to carry all this weight. Menopause really did a doosey on me. (😶lol) My daughter also lives in Houston. with her Husband and my 5 grand-littles. I grew up in Beaumont, so I know Houston well, I will be sure to keep in touch and update you on my journey. I may need some advice in the future, or just motivation.       Soooo I am coming to a realization
      of something and I'm not sure what to do about it. For years the only thing I've enjoyed is eating. We rarely do anything or go anywhere and if we do it always includes food. Family comes over? Big family dinner! Go camping? Food! Take a short ride or trip? Food! Holiday? Food! Go out of town for a Dr appointment? Food! When we go to a new town we don't look for any attractions, we look for restaurants we haven't been to. Heck, I look forward to getting off work because that means it's almost supper time. Now that I'm drinking these pre-op shakes for breakfast, lunch, and supper I have nothing to look forward to.  And once I have surgery on June 11th it'll be more of the same shakes. Even after pureed stage, soft food stage, and finally regular food stage, it's going to be a drastic change for the rest of my life. I'm giving up the one thing that really brings me joy. Eating. How do you cope with that? What do you do to fill that void? Wow. Now I'm sad.

        I hope your surgery on Wednesday goes well. You will be able to do all sorts of new things as you find your new normal after surgery. I don't know this from experience yet, but I am seeing a lot of positive things from people who have had it done. Best of luck!

        Life as a big person had limited my life to what I knew I could manage to do each day. That was eat. I hadn't anything else to look forward to. So my eating choices were the best I could dream up. I planned the cooking in managable lots in my head and filled my day with and around it.

        Now I have a whole new big, bigger, biggest, best days ever. I am out there with those skinny people doing stuff i could never have dreamt of. Food is now an after thought. It doesn't consume my day. I still enjoy the good home cooked food but I eat smaller portions. I leave food on my plate when I am full. I can no longer hear my mother's voice saying eat it all up, ther are starving children in Africa who would want that!

        I still cook for family feasts, I love cooking. I still do holidays but I have changed from the All inclusive drinking and eating everything everyday kind to Self catering accommodation. This gives me the choice of cooking or eating out as I choose. I rarely drink anymore as I usually travel alone now and I feel I need to keep aware of my surroundings.

        I don't know at what point my life expanded, was it when I lost 100 pounds? Was it when I left my walking stick at home ? Was it when I said yes to an outing instead of finding an excuse to stay home ? i look back at my last five years and wonder how loosing weight has made such a difference. Be ready to amaze yourself.

        BTW, the liquid diet sucks, one more day and you are over the worst. You can do it.
