50/50 chance of esophageal dilation - really upset

I got the band 5 years ago but then was a total failure. I didn't follow the rules very well, I was pregnant, etc. I started again in January. Since January I have lost over 40lbs. My doctor told me (he does all the fills under the flourosope) in Janauary and again a month later that the band was in perfect condition and the perfect location. The problem he had was my stomach spasms continuously so it was really hard to fill it and gauge how well it was filled. I am on some medicine for the stomach spasms but it's not working that well.

I went to get another fill the other day because i see him monthly and he had not filled it since the original fill because I was losing weight. My weightloss has slowed a bit and I did not have as much restriction. He told me the band is now more vertical and I have a 50% chance of esophageal dilation. I don't really understand what that is. I kind of get it but it still doesn't make a lot of sense. I asked him if he could just move the band back but he said that they used to do that but now that doesn't work or isn't that effective. So far it has not dilated - I have restriction. But if I lose restriction I am going to be so upset. I asked him about revision - he is SUPER conservative. Absolutely will not do revisions unless I meet very stringent criteria. I think he is trying to build a record that he has done everything with the band for insurance purposes (i.e. I am going to the dietician a lot). It doesn't help that I am on the low end of the weight spectrum - my BMI is 35 I believe - so aside from PCOS, my other co-morbidity (sleep apnea) has disappeared.

Has this happened to anyone else? what is it exactly? how did your doctor solve it? I was told he would unfill it and I would know if it dilated because I would lose restriction. But what happens then? I just have a non-working band in me the rest of my life? Can't that cause erosion? I am just confused and distressed. I didn't really know what to ask my doctor other than was I in any kind of danger................

I'm very sorry you're dealing with this.

You need to have your doctor explain two things to you: How can it be fixed and what can you do to help fix it.

I am shocked he didn't completely unfill your band. Right now, you need to be concerned with the esophageal dilation not whether or not you have "restriction". This is especially true because of one of the causes of esophageal dilation is having a band too tight. Another cause is chronic over-eating. Being as you already said you weren't compliant with your band it's really important that you are given clear instructions what to do now and follow them exactly.

Here's portion of an article that covers esophageal dilation:

Esophageal and gastric pouch dilatation have been reported in many series [6,18–23]. The most likely cause of dilatation is over inflation of the LAP-BAND combined with patients who do not comply with instructions regarding oral intake. Inappropriate intake can stretch the gastric pouch; eventually, the pouch, and even the esophagus, may dilate as occurs in other restrictive procedures. Although these changes are usually reversible if the problem is detected sufficiently early, failure to address the issue will result in an atonic pouch and, potentially, a large, atonic esophagus.Gastric pouch dilatation should be viewed as a preprolapse condition that can be alleviated with prompt diagnosis and treatment. Its management typically involves complete deflation of the band and, after 2 to 3 months, slow re-inflation,with the surgeon being careful not to reach the previous point of over inflation.

Get back into your doctor. It's really important you get proactive with this before a fixable problem is left untreated and becomes something that costs you your band.

thank you for your reply. My doctor didn't seem that concerned and said if it does dialate then he will unfill the band....I do think it is too tight and I will call Monday to get in to have it unfilled somewhat. I am just so frustrated right now.....

Dilation alone isn't the huge concern, well most of the time, but can be sign of ulceration or perforation later on. I had dilation continued to vomit then ended up perforating. I just recommend continuing to update your doctor on symptom as I did not do thirs like I should have done

      I am new here today... and only two weeks out from my sleeve surgery on the 23rd. I am amazed I have kept my calories down to 467 today so far... that leaves me almost 750 left for dinner and maybe a snack. This is going to be tough for two weeks... but I have to believe I can do it!

      Hey everyone. I'm new here so I thought I should introduce myself. I am 53y/o and am scheduled for Gastric Bypass on June 25th, 2025. I'm located in San Antonio, Texas. I will be having my surgery in Tiajuana Mexico. I've wanted this for years, but I always had insurance where bariatric procedures were excluded. Finally I am able to afford to pay out of pocket.  I can't wait to get started, and I hope I'm prepared for the initial period of "hell". I know what I have signed up for, but I'm sure the good to come will out way the temporary period of discomfort and feelings of regret. I'd love to find people to talk to who have been through the same procedure or experience before. So I look forward to meeting you all. Hope you have a great week!

      On day 4 of the 2 week liquid pre-op diet. Surgery scheduled for June 11th.
      Soooo I am coming to a realization
      of something and I'm not sure what to do about it. For years the only thing I've enjoyed is eating. We rarely do anything or go anywhere and if we do it always includes food. Family comes over? Big family dinner! Go camping? Food! Take a short ride or trip? Food! Holiday? Food! Go out of town for a Dr appointment? Food! When we go to a new town we don't look for any attractions, we look for restaurants we haven't been to. Heck, I look forward to getting off work because that means it's almost supper time. Now that I'm drinking these pre-op shakes for breakfast, lunch, and supper I have nothing to look forward to.  And once I have surgery on June 11th it'll be more of the same shakes. Even after pureed stage, soft food stage, and finally regular food stage, it's going to be a drastic change for the rest of my life. I'm giving up the one thing that really brings me joy. Eating. How do you cope with that? What do you do to fill that void? Wow. Now I'm sad.

        I hope your surgery on Wednesday goes well. You will be able to do all sorts of new things as you find your new normal after surgery. I don't know this from experience yet, but I am seeing a lot of positive things from people who have had it done. Best of luck!

        Life as a big person had limited my life to what I knew I could manage to do each day. That was eat. I hadn't anything else to look forward to. So my eating choices were the best I could dream up. I planned the cooking in managable lots in my head and filled my day with and around it.

        Now I have a whole new big, bigger, biggest, best days ever. I am out there with those skinny people doing stuff i could never have dreamt of. Food is now an after thought. It doesn't consume my day. I still enjoy the good home cooked food but I eat smaller portions. I leave food on my plate when I am full. I can no longer hear my mother's voice saying eat it all up, ther are starving children in Africa who would want that!

        I still cook for family feasts, I love cooking. I still do holidays but I have changed from the All inclusive drinking and eating everything everyday kind to Self catering accommodation. This gives me the choice of cooking or eating out as I choose. I rarely drink anymore as I usually travel alone now and I feel I need to keep aware of my surroundings.

        I don't know at what point my life expanded, was it when I lost 100 pounds? Was it when I left my walking stick at home ? Was it when I said yes to an outing instead of finding an excuse to stay home ? i look back at my last five years and wonder how loosing weight has made such a difference. Be ready to amaze yourself.

        BTW, the liquid diet sucks, one more day and you are over the worst. You can do it.
