Issues with obtaining past weight loss documentation

I have all of the documentation required for the surgery but I would like to have additional documentation from a past weight loss center that I used. Has anyone been able to obtain paperwork from the LA Weightloss Center group? They are missing in action here and I can not contact any number for them on the net.

If anyone could help that would be wonderful!!

Or if you have any suggestions to help as well.

Oh yea, I went through this! You can contact the corporate office, which is still open but NO help! Luckily, I still had my contract from them and my insurance company took that as proof along with a letter that I wrote explaining how they had gone out of business. I know, it is very frustrating, as I went through the exact same thing! If you would like to contact the corporate office, you can get thier number of the main website still.

thank you for the information. Can I ask who your insurance carrier is? I hope mine will be the same way and I have letters from my Doctor as well stating that they went out of business and we can not obtain records.

I had this issue with Slim4Life, which is a very similar company to L.A. Weight Loss. I had put my program on hold (I'd been going for over a year) because I got pregnant. I called them last year when I decided to get the lapband so that I wouldn't have to go through six months of dieting, but they had none of my records. I called corporate and they had DESTROYED my records and couldn't do anything about it. They claim they do that so your personal information won't be lying around. So I had to go through six months of supervised dieting with my doctor, which I just finished. Now I'm just gathering up the last of my paperwork to submit to insurance.

Soooo...I had no luck getting my past weight loss records...hope you can do better than I did!

Could I get a copy of each of your letters to your insurance companies? I would like one to mine and I am not sure as to what to state in the letter.

why do we need to prove we have been on a diet for 6 months, or how ever long they requier? I don't know about your insurance but mine also requires I lose 5% of my weight, which I did not. So they denied me. I'm going to appeal and if they still deny me I will take to the State Insurance Commission. Do you know my insurance will approval and pay 100% (except $150 copay) for a male penel implant which is $60,000 plus. No require ments nothing to prove, just have your doctor submit. Well I'm here to tell you that's discrimination and I'm hopping mad! I will not quite till I win and believe me I will win! good luck ladies. bb

I went to WW for a year and had thrown away my card with all my weights on it. I have BCBS and I got copies of my bank statement showing my payments to them for that period of time to prove I was going. They were fine with that as proof. Just a thought.

I have BCBS of MI do you think it will be the same? I gave my surgeon's office a copy of cc statements with charges along with copies of my gym membership payments for the year 2008. I have to go for my appointment with the psychiatrist on Friday and that should be the last item required by the surgeon. Lets hope everything runs smoothly.

      Question for anyone, how did you get your mind right before surgery? Like as far as eating better foods and just doing better in general? I'm having a really hard time with this. Any help is appreciated 🙏❤️

        I had about 6 months between deciding to do surgery and getting scheduled. I came across the book The Pound of Cure by Dr. Matthew Weiner, a bariatric surgeon in Arizona, and started to implement some of the changes he recommended (and lost 13 lbs in the process without ever feeling deprived). The book is very simple, and the focus is on whole, plant based foods, but within reason. It's not an all or nothing approach, or going vegan or something, but focuses on improvement and aiming for getting it right 80-90% of the time. His suggestions are divided into 12 sections that you can tackle over time, perhaps one per month for a year if a person is just trying to improve nutrition and build good habits. They range from things like cutting out artificial sweetener or eating more beans to eating a pound of vegetables per day. I found it really effective pre-surgery and it's an eating style I will be working to get back to as I am further out from surgery and have more capacity. Small changes you can sustain will do the most for building good habits for life.

      I've hit a stall 9 months out. I'm not worried, though. My fitness levels continue to improve and I have nearly accomplished my pre-surgery goal of learning to scuba dive! One dive left to complete to get my PADI card 🐠
      I was able to go for a 10K/6mile hike in the mountains two days ago just for the fun of it. In the before days, I might have attempted this, but it would have taken me 7 or 8 hours to complete and I would have been exhausted and in pain for the next two days. Taking my time with breaks for snacks and water, I was finished with my wee jaunt in only 4 hours 😎 and really got to enjoy photographing some insects, fungi, and turtles.
      Just for fun last week, I ran two 5Ks in two days, something I would have never done in the past! Next goal is a 10K before the end of this month.

      Hi everyone, I wrote back in May about having no strength. I still get totally exhausted just walking from room to room, it’s so bad I’m using a walker with wheels of all things. I had the gastric sleeve Jan. 24th. I’m doing exactly what the programs says, except protein shakes. I have different meats and protein bars daily, including vitamins daily. I do drink my fluids as well.  I go in for IV hydration 4 days a week and feel ok just til evening.  So far as of Jan 1st I’ve dropped 76 lbs. I just want to enjoy the weight lose. Any suggestions or has anyone else gone thru this??  Doctor says just increase calorie intake, still the same.

      Losing my hair in clumps and still dealing with "stomach" issues from gallbladder removal surgery. On the positive side I'm doing better about meeting protein and water goals and taking my vitamins, so yay? 🤷‍♀️
