Just converted my Sleeve to the SADI-S and have millions of questions....

Recommended Posts

I was originally sleeved 9/2010 and lost 105. The past 2 years I regained about 40 of those pounds and was just not okay with it. On 8/11/2016 I had the intestinal portion of the SADI-S completed- my sleeve was still in good shape and needed no revision.

I'm now 8 days post-op from my SADI-S Revision and down 12 pounds (I know, holy crap). Because previously I had the sleeve, that is the only reference I have to go on in terms of what to expect post-op. However, it's become clear to me that this "intestinal" revision is NOTHING like getting sleeved. I have NO issues with drinking, swallowing, topping off, or stomach restriction associated pains. I do seem sensitive to some things, like drinks or food that have sugar added, but it's still too early for me to know all the likely sensitivities. Also I've only had 2 pooty bouts, and yes, quite a bit of flatulence. And this is part of that unknown for me.

What gives u gas? How many calories should I be consuming? What's gonna make me pooty, or throw up, or have horrible diarrhea? Does the gas/pooty/diarrhea happen right after I eat something or does it take a few hours, a day? Should I be focusing on fats as well as Proteins? Someone I follow on YT said they eat an avg of 3000 cals a day? Is that just for DS, that's what she had, is that for all DS? What foods should I avoid like the plague? Anyone else have the revision done with no change to their sleeve? How did u deal with that mentally, that everything is pretty much the same as before u went into surgery? How much should I be eating? Any daily caloric goal I should be reaching for? What happens when I do transgress and eat a sandwich with the bread, or 4 fries outta my daughters Wendy's bag? How's my body gonna react?

Sorry y'all, I know I really went on, but this is soooo not like my VSG that it's just left me at a loss. I don't know what to expect or anticipate anymore. Thanks ahead for any advice u share.

I don’t know what to expect or have many answers to your questions because I am in the exact situation as you are but I am able to provide some support. I hope this will help…

I was sleeved July 2011 and at a weight of 315 pounds. I got down to 180 pounds by December 2013. I began having severe asthma problems in Aug of 2014. Over the course of little over a year from Aug 2014 to February 2016 I was required to have monthly steroid injections in order to control my asthma. I was not able to exercise adequately without having an asthma attack. I gained weight rapidly. Within that time I ballooned back up to 260 pounds. March 2016 my asthma doctor put me on my “miracle injection” NUCALA and my asthma is now totally controlled!!

On 8/8/2016 I had the intestinal portion of the SADI-S completed with a different surgeon and just like you my sleeve was still in good shape and needed no revision. I am the first patient that my doctor has performed the SADI S Loop on and he really does not have reference to go by. I will be his first reference going forward. I agree with you, this "intestinal" revision is NOTHING like getting sleeved. I also have NO issues with drinking, swallowing, topping off, or stomach restriction associated pains. Since the sleeve portion was done previously my surgeon is allowing me to progress through the food stages sooner that someone who just had a DS or sleeve. At two weeks, Aug 19, I was progressed to soft foods. In two more weeks I can progress to full foods. I am taking this time slowly. I am sensitive to certain drinks and foods. I have to eat slowly and pay close attention to my body in order to gauge if I can eat or drink things. Smells, textures, and certain foods and drinks will make me nauseous. Protein drinks give me HORRIBLE gas. I have a daily potty bout. Every time I introduce something new into my diet I make sure I am at home just in case I need to make a mad dash for the potty. My surgeon told me that some individuals who have had the SADI S Loop repot less frequent smelly flatulence and some Constipation. I have yet to experience that part. He explained that it is due to the length of the inessential tract that has been left.

I am currently on the same caloric restriction as I was when I had my VSG years ago. The only difference has been my length of time on liquids and soft foods. In order for me to adequately loose I my calories will need to remain around 1000 and I will need to exercise daily. I don’t have any idea what the future holds as far as when we progress to foods with complex carbs in them. I personally am so afraid of winding right back at 250 that I don’t plan on introducing them back in. Don’t get me wrong, I love chocolate CAKE. But this time I plan on cooking a low carb cake for the family instead of buying and eating the one from Fresh Market.

Congratulations on your revision. It is totally scary but we can do this!!!

Thanks so much for ur reply!!! I am doing much better now, thankfully. I'm now nearly 3 weeks out and my body is adapting (I forgot about that), and getting into a pooty-rhythm, if that is a thing. I now pooty about 3 times a day - morning, afternoon, and night. The gas has decreased dramatically and I'm getting a good sense for what I should and should not be eating and how much of some of the should nots I can get away with .

Please update me on how u are now doing! I'd luv to hear if ur body too is now "settling in". And thanks for ur support, it really made me feel better.

I am so glad that you are feeling so much better and that your body is adjusting. I'm doing a lot better these days myself. I actually am beginning to experience some Constipation. I began taking a liquid supplement by Bluebonnet called Calcium Magnesium Citrate Plus Vitamin D3. It helps me tremendously by keeping me regular and soft. I take it every evening along with my evening Vitamins and medications.

I have progressed to solid foods and have found that some things don't agree with me as the once did. I used to love Peanut Butter. I have found that now it's too sweet and too oily. I can't stomach red meats either. Carbmaster has a new low carb smoothie that I tried today and it didn't sit well. I think the smoothie was too thick. I can still eat Carbmaster low carb yogurt with a scoop of added Protein.

As of this past Monday 29AUG16 I started going back to the YMCA so I could swim in the early mornings. Well.. I started off doing Water aerobics for thirty minutes and swimming for ten minutes before I got tired. I have been going every day this week and have been increasing my endurance. I now do Water aerobics for twenty minutes and swim for twenty minutes. When I get cleared by the surgeon I will begin to add some light weight lifting on Monday, Wednesday, and Friday so I wont loose all on my fat free muscle mass. I will see him in two more weeks.

I must say that my weight loss has been considerably slower. My surgeon had prepared me by informing me that the weight loss would be slower than before. I just need to relax and let the process work no matter how long it takes. I'm not competing with anyone just my preconceived ideas of how this should work for me. I'm glad that you did read my response and know that I am here if you need me. Truthfully I need as much support as I can get especially since this is a revision and I am deathly afraid of failing!!. So please keep me posted of your progress as well. I really appreciate knowing that there is someone else on this board who has a similar background as I do. I don't feel so alone anymore. Thanks :)

I'm feeling I on all the stuff u notes girl. I've been pretty lucky so far with the weight loss, but last time my body also responded really well to the surgery. It's of course coming off slower than last time, but a million times faster than when I kept trying and failing over and over again to lose the damn regain on my own. I'll be 1 month out in 3 days and I've dropped 17 pounds....very happy with that. A contributing factor might have been that I was back on soft solids after 1 week, not my doctors advice, but I knew my tummy hadn't changed, I just needed to be careful with my intestines.

I can't begin to express again how much ur feedback and experiences are appreciated. I've had literally no one to talk to who had this revision with no change to their tummy so I've had to make everything up as I've gone along. So u taking the time to share ur experience with me is literally invaluable.

Oh, I've been vlogg in my entire journey since 2010 on YouTube (cecelialane) ...check it out so we can "meet" and email me anytime at cecelia@wlssuccesswithcecelialane.com.

I'm also on IG, ooVoo, Facebook (Cecelia Brown Lane) and Messenger

So happy things are improving for u too. Now I'm pootying primarily just in the mornings, albeit about 6 times. Buta till, it's a relief.

Question for anyone, how did you get your mind right before surgery? Like as far as eating better foods and just doing better in general? I'm having a really hard time with this. Any help is appreciated 🙏❤️

I had about 6 months between deciding to do surgery and getting scheduled. I came across the book The Pound of Cure by Dr. Matthew Weiner, a bariatric surgeon in Arizona, and started to implement some of the changes he recommended (and lost 13 lbs in the process without ever feeling deprived). The book is very simple, and the focus is on whole, plant based foods, but within reason. It's not an all or nothing approach, or going vegan or something, but focuses on improvement and aiming for getting it right 80-90% of the time. His suggestions are divided into 12 sections that you can tackle over time, perhaps one per month for a year if a person is just trying to improve nutrition and build good habits. They range from things like cutting out artificial sweetener or eating more beans to eating a pound of vegetables per day. I found it really effective pre-surgery and it's an eating style I will be working to get back to as I am further out from surgery and have more capacity. Small changes you can sustain will do the most for building good habits for life.

That sounds awesome. I'll have to check that out thanks!

I've hit a stall 9 months out. I'm not worried, though. My fitness levels continue to improve and I have nearly accomplished my pre-surgery goal of learning to scuba dive! One dive left to complete to get my PADI card 🐠
I was able to go for a 10K/6mile hike in the mountains two days ago just for the fun of it. In the before days, I might have attempted this, but it would have taken me 7 or 8 hours to complete and I would have been exhausted and in pain for the next two days. Taking my time with breaks for snacks and water, I was finished with my wee jaunt in only 4 hours 😎 and really got to enjoy photographing some insects, fungi, and turtles.
Just for fun last week, I ran two 5Ks in two days, something I would have never done in the past! Next goal is a 10K before the end of this month.

Hi everyone, I wrote back in May about having no strength. I still get totally exhausted just walking from room to room, it’s so bad I’m using a walker with wheels of all things. I had the gastric sleeve Jan. 24th. I’m doing exactly what the programs says, except protein shakes. I have different meats and protein bars daily, including vitamins daily. I do drink my fluids as well.  I go in for IV hydration 4 days a week and feel ok just til evening.  So far as of Jan 1st I’ve dropped 76 lbs. I just want to enjoy the weight lose. Any suggestions or has anyone else gone thru this??  Doctor says just increase calorie intake, still the same.

Losing my hair in clumps and still dealing with "stomach" issues from gallbladder removal surgery. On the positive side I'm doing better about meeting protein and water goals and taking my vitamins, so yay? 🤷‍♀️
