Insulin Resistant and PCOS - 1st month post surgery and not losing weight

Hello.

I had been looking for forums online to share my experience and get views from people going through similar experiences. I am from Pakistan and weight loss surgeries are not too common here. I was the 60th sleeving patient in 3 years for my Surgeon.

So i am exactly 4 weeks post sleeve and 8 kgs down from the weight on the day of my surgery. I lost 2 kgs in the first week, 5 in the second week and half a kg each in the 3rd and 4th week. We accidentally discovered that i was hypothyroid and had poly cystic ovaries, 3 to 4 days before the surgery(i decided on having the surgery on a very short notice - only a week before and there were 4 of us who had a sleeving surgery on the same date). 2 days post surgery while still hospitalized with TSH level 5.75 i was prescribed 50 mcg of thyroxine everyday.

On my second visit to the surgeon after the surgery, my doctor recommended a Thyroid test follow up to figure why i was not losing weight as rapidly as the other sleeving patients who had the surgery on the same date. They have lost about 15 to 18 kgs and are all about 10 to 14 years older to me. My current TSH level has fallen down to 0.023. I have to meet my endocrinologist day after however i have been quite upset since my TSH reports came. I dont understand how i went to being hyperthyroid from hypothyroid. Also, doesnt hyperthyroidism make you lose weight drastically? Does PCO hinder weight loss?

I would appreciate if any of you could get back. Any suggestions? Opinions?

Sorry you have been having some problems. Here in the U.S. we are required to have a lot of blood work done before surgery. This includes thyroid tests to make sure our thyroid is working properly. I was prescribed thyroid medicine 2 years ago (I don't remember if I was hypo or hyper) anyway, I hadn't been taking that medication and when I went for my recent test my thyroid was fine. I hope you get leveled out and start seeing the weight loss you are hoping for.

Hi, I'm sorry to hear about your difficulties. I have polycystic ovarian syndrome and insulin resistance as well. Yes, it makes it more difficult to lose weight. From my limited experience the best thing you can do is eat high Protein diet and get lots of exercise. My doctor told me that once my insulin resistance went away my weight will fall off pretty fast. I'm due to have my sleeve done next month. I'm also currently on a diet and have noticed some differences. A large majority of women with pcos can't tolerate gluten well. So if you eat a lot of breads that could make you gain weight rapidly. You are also brand new to thyroid meds. I've been on them over 20 years and have learned the hard way that you need to be on a certain dose for 6-8 weeks to see how your body handles it. At first, mine got adjusted every few months until the levels were good. The meds do not really help you lose weight, my dr said basically they may give you a 5-10 lb loss and the rest is up to you. Being hyperthyroid doesn't mean you will lose weight either. My level is currently .01 (we are working on bringing it up) and I lost nothing from that. It's been there about 6 months. My body feels the best at about a 1 so that's where my dr aims for even though some other doctors would call that hyperthyroid.

I also have pros and insulin resistance, those are mostly caused by being overweight and will at least partially resolve themselves just by losing weight. It's kind of a vicious circle!

I don't know kg but I've been sleeved 2-1/2 weeks with a very high starting weight, BMI of 55 and I've only lost 6 lbs so far. We are all so different. Keep your chin up!

Yes i have been having 1000 mg of Metformin everyday. My surgeon also prescribed Orlistat recently. Not sure if that has been the reason of anxiety and depression.

No, the orlistat shouldn't cause depression. Depression and anxiety are a symptom of pcos. Are you on birth control pills? Spirlactone? Spirlactone blocks androgens, which helps to balance hormones. It's a Fluid pill. Once I started taking it, I noticed my symptoms improved.

I am also a slow loser too! Ugh

Pre surgery I was taking insulin NPH relics a day ; actos pills 4 a day and Metformin 2000 Mgs a day

Now I'm post op I ONLY take Metformin one on am one in pm-- this is awesome!! So glad I got sleeved !

My Sw was 283 I am now 237

I only take levothyroxine and Metformin that's it

I do have pcos - I don't ovulate on my own at all

That's why I had surgery bcuz I want a baby!

Been married 10 years and no luck! My ob/gym suggested I get the surgery

I have hypothyroid and pros. I'm insulin resistant as well. I was sleeved 3/31. My tsh levels have been pretty stable until about a month before surgery. It shot up to 11. I'm not sure when it has ever been so high. I could tell that it came down though because of the way I was feeling.

Hi Ladies,

I haven't been sleeved yet but do suffer from Hoshimoto's disease, PCOS and insulin resistance. I just want to throw out my two cents. I have been dealing with it for about 14 years and was Levothyroxine and Synthroid...metformin....you name it. I never felt good. The thyroid symptoms were unbearable even though my labs were within "normal range". I finally was fed up with 'umpteen' Endocrinologist s and even my original PCP. I researched everything and came across Stop the Thyroid Madness http://www.stopthethyroidmadness.com/. They mentioned T3 and T4 and all this stuff I had never been tested for. Then they mentioned Armour Thyroid as a medication. It is made from pig thyroid and had to be perscribed. My PCP refuse to prescribe anything other than the two main name brands so I fired him. I have since be on Armour for almost 3 years and it changed my life. I have the occasional flare but it has been the difference between night and day. Before I was sooooooo fatigued I slept through my 1 year old crying in his crib in the morning...I would wake up at 11am when he had been awake since 8....it was the last straw. I also found a doctor that treats based on SYMPTOMS not blood test alone. NOT EVERYONE feels great at 3.5 even though its within " normal range". Be your best advocate. I went through 4 docs until I landed on my current PCP of 3 years.

Another big contributor....processed food of any kind. Whole organic foods have made a HUGE difference. I've lost about 13lbs without changing anything but quality of food. If you would like more info PM me....I don't want it to seem like I am pushing websites or anything...just information.

Hello girls. Logged in after 2 weeks today and glad to see all your replies.. i stopped taking thyroxine to balance my insulin level.. will probably run another tsh test in 2 weeks after which my endocrinologist will decide if i really need thyroxine artificially. My doctor however increased my metformin dosage to 1.5g per day.

I lost another 5 kgs in 2 weeks. So glad the scale finally started moving. Ive been brisk walking 1 hour a day and swimming 30 mins, 3 days a week... totally looking forward to a better and healthier life :D

I have had hypothyroidism for about 20 years. I recall that it took some time to get the right dosage for me. Also, after any major medical event (childbirth, endometriosis, hysterectomy), we would have to zero in on a new dosage which just takes time. I wonder if the depression is part of the thyroid issue and will resolve itself when the dosage is correct. I find that when the dosage is off I feel fatigued, depressed, dry skin, lots of stuff you would not think was related to that little fella.

Hang in there. You are doing everything you can and it will work its way out. However, it will probably take more time. Good Luck!

      2 days until I fly out to San Diego to have my Bypass Surg. in Tiajuana Mexico. Not gonna lie, the nerves are starting to surface. I don't fear the surgery itself, or the fact that I'm traveling alone, but its the aftermath that I'm stressing about the most, after this 8 week wait. I'm excited to finally be here, but I am really dreading the post surgical chapter. I know its going to be tough, real tough and I think I'm just in my head to much now that the day i here. Wish me luck, Hopefully I'm one of the lucky ones, and everything goes smoothly. Cant wait to give an exciting update,. If there is anyone else have a June bypass or even a recent one, Id love to have someone to compare war stories with. Also, anyone near San Antonio Tx? See ya soon with the future me. 💜

      I am new here today... and only two weeks out from my sleeve surgery on the 23rd. I am amazed I have kept my calories down to 467 today so far... that leaves me almost 750 left for dinner and maybe a snack. This is going to be tough for two weeks... but I have to believe I can do it!

      Hey everyone. I'm new here so I thought I should introduce myself. I am 53y/o and am scheduled for Gastric Bypass on June 25th, 2025. I'm located in San Antonio, Texas. I will be having my surgery in Tiajuana Mexico. I've wanted this for years, but I always had insurance where bariatric procedures were excluded. Finally I am able to afford to pay out of pocket.  I can't wait to get started, and I hope I'm prepared for the initial period of "hell". I know what I have signed up for, but I'm sure the good to come will out way the temporary period of discomfort and feelings of regret. I'd love to find people to talk to who have been through the same procedure or experience before. So I look forward to meeting you all. Hope you have a great week!

        I'm so happy for you! You are about to change your life. I was so glad to get the sleeve done in Dec. I didn't have feelings of regret overall. And I'm down almost 60 lbs. I do feel a little sad at restaurants. I can barely eat half a kid's meal. I get adults meals often because kid ones don't have the same offerings at times. Then I feel obligated to eat on that until it's gone and that can be days. So the restaurant thing isn't great for me. All the rest is fine by me! I love feeling full with very little. I do wish I could drink when eating. And will sip at the end. Just a strong habit to stop. But I'm working on it! You will do fine! Just keep focused on your desire to be different. Not better or worse. But different. I am happy both ways but my low back doesn't like me that heavy. So I listened (also my feet!). LOL! Update us on your journey! I'm not far from you. I'm in Houston. Good luck and I hope it all goes smoothly! Would love to see pics of the town you go to for this. I've never been there. Neat you will be traveling for this! Enjoy the journey. Take it one day at a time. Sometimes a few hours at a time. Follow all recommendations as best you can. 💗

        Thank you so much for your well wishes. I am hoping that everything goes easy for me as well. We don't eat out much as it is, so it wont be too bad in that department. Thankfully. Also, I hear you regarding your back and feet!! I'd like to add knees to the list. Killing me as we speak! I'm only 5' so the weight has to go. Too short to carry all this weight. Menopause really did a doosey on me. (😶lol) My daughter also lives in Houston. with her Husband and my 5 grand-littles. I grew up in Beaumont, so I know Houston well, I will be sure to keep in touch and update you on my journey. I may need some advice in the future, or just motivation. Thank You so much for reaching out, I was hoping to connect with someone in the community. I really appreciate it. 💜

      On day 4 of the 2 week liquid pre-op diet. Surgery scheduled for June 11th.
      Soooo I am coming to a realization
      of something and I'm not sure what to do about it. For years the only thing I've enjoyed is eating. We rarely do anything or go anywhere and if we do it always includes food. Family comes over? Big family dinner! Go camping? Food! Take a short ride or trip? Food! Holiday? Food! Go out of town for a Dr appointment? Food! When we go to a new town we don't look for any attractions, we look for restaurants we haven't been to. Heck, I look forward to getting off work because that means it's almost supper time. Now that I'm drinking these pre-op shakes for breakfast, lunch, and supper I have nothing to look forward to.  And once I have surgery on June 11th it'll be more of the same shakes. Even after pureed stage, soft food stage, and finally regular food stage, it's going to be a drastic change for the rest of my life. I'm giving up the one thing that really brings me joy. Eating. How do you cope with that? What do you do to fill that void? Wow. Now I'm sad.

        I hope your surgery on Wednesday goes well. You will be able to do all sorts of new things as you find your new normal after surgery. I don't know this from experience yet, but I am seeing a lot of positive things from people who have had it done. Best of luck!

        Life as a big person had limited my life to what I knew I could manage to do each day. That was eat. I hadn't anything else to look forward to. So my eating choices were the best I could dream up. I planned the cooking in managable lots in my head and filled my day with and around it.

        Now I have a whole new big, bigger, biggest, best days ever. I am out there with those skinny people doing stuff i could never have dreamt of. Food is now an after thought. It doesn't consume my day. I still enjoy the good home cooked food but I eat smaller portions. I leave food on my plate when I am full. I can no longer hear my mother's voice saying eat it all up, ther are starving children in Africa who would want that!

        I still cook for family feasts, I love cooking. I still do holidays but I have changed from the All inclusive drinking and eating everything everyday kind to Self catering accommodation. This gives me the choice of cooking or eating out as I choose. I rarely drink anymore as I usually travel alone now and I feel I need to keep aware of my surroundings.

        I don't know at what point my life expanded, was it when I lost 100 pounds? Was it when I left my walking stick at home ? Was it when I said yes to an outing instead of finding an excuse to stay home ? i look back at my last five years and wonder how loosing weight has made such a difference. Be ready to amaze yourself.

        BTW, the liquid diet sucks, one more day and you are over the worst. You can do it.
