This is thru the obesity coalition.. you have to have your drs office fill out the form for the most part and fax it in.. BUT you get a huge bottle of calcuim citrate chewables (500 i think).. Vitamins.. b12 sublingual.. and Iron. All thru Bariatric Advantage and bariatric approved. Did this today for myself.

Maybe u have to do it at a computer.. the link takes u directly to the page u download it from. Ull have to print it then fill it out
