Criteria?

I am going in to my PCP next Monday and I am going to be requesting the referral for the bariatric program hoping to be accepted for it - I am leaning toward the sleeve but what are the criteria they go by - I am 245lbs - 5'6" and my BMI is at 39 - does anyone know what the BMI requirements are?
